                        All information, including current contact phone numbers, must be completed before child
                             is allowed to participate in the program.
 

                   HOURS OF OPERATION:
                              Mornings only
                               7:00 a.m. until school opens
                               All days school is in operation
                               Not in operation on 2 hour delays
